An 8D is the eight-discipline problem-solving report an OEM expects after a quality escape, and a weak 8D is one of the fastest ways to end up on controlled shipping. The report is not paperwork for its own sake; it is how you prove to the customer that the problem is understood, contained, and permanently fixed.
The eight disciplines, briefly
The 8D runs from D1 to D8: build the team, describe the problem, put in interim containment, find and verify the root cause, choose and verify permanent corrective actions, implement them, prevent recurrence, and recognize the team. The heart of it is the middle: real root cause and permanent, verified corrective action.
What the OEM actually checks
Reviewers look past the format for a few things: proof that containment was immediate and complete, a root cause that actually explains the escape rather than a symptom, corrective actions that are irreversible instead of temporary sorting, and evidence that the fix was verified to work. If any of those is thin, the 8D comes back.
Common reasons an 8D gets rejected
The usual failures are containment that does not cover parts in transit and at the customer, a shallow root cause, corrective actions that rely on inspection rather than a process fix, and no verification data.
